After following Yuu into the mansion, Rei is chased by a spirit in the hall while exploring the mansion's entrance. The spirit is covered in tattoos, and after a brief touch, Rei wakes up from a nightmare, only to find the same tattoos beginning to appear on her body, causing her pain. Every time Rei falls asleep, she returns to the mansion each night to search for the truth behind the cursed tattoos. Eventually, she discovers that only one survivor of the disaster is brought to the snow-covered mansion.

Miku Hinasaki, who lives with Rei and works as her assistant, is the sole survivor from the Himuro Mansion in Fatal Frame, and she too is drawn into the nightmare. With her help, Rei uncovers a dark ritual that took place within the mansion, and eventually, the fated demise of the mansion's residents is revealed. The two must work quickly to eliminate the darkness that haunts the mansion before the curse of the tattoos consumes them, trapping them in the nightmare forever.
